This should be done in the last two weeks of school and optional during midyear to find out what the students think of you as a teacher, what is working well in class, and if there should be any modifications for future classes. Remember this needs to be anonymous for the students, and should only be used to make sure that you become a more effective teacher, not to be taken personally if the students hated your class.
